Step 1: Get the Form
Obtain the “NDIS Reviewable Decision Form” to request a review of your NDIS plan.
Step 2: Choose Your Method
Decide how you want to submit your request:
- Visit a NDIS office and talk to someone in person. You can search for an office near you here.
- Call the NDIS hotline at 1800 800 110.
- Email the form to [email protected].
- Mail the form to the Chief Executive Officer, National Disability Insurance Agency, GPO Box 700, Canberra ACT 2601.
Step 3: Prepare for Your Meeting
If your early review request is approved, it’s time to get ready:
- Think about how things have changed for you since your last review.
- Write down any new goals or challenges you have.
- Gather any papers or reports that show why you need a review.
- Figure out what help or services you need to reach your goals.
